Through the Vital Health Plan, we offer you access to a Model of Care that integrates physical and mental health, under the care of a team of providers focused on helping you improve your health. First Medical has a wide and robust network of health care providers distributed throughout the island. You can visit general practitioners, family practitioners, specialists, sub-specialists, dentists, hospitals, health centers, emergency rooms, laboratories and radiology centers, selecting the service of your preference using our Provider Directory. First Medical has alternative methods available to provide you with important information about the services we offer, at no cost to you. These include, but are not limited to, Braille, enlarged print and translation into other languages, verbal or written. The Government of Puerto Rico’s Health Plan, Vital is responsible for providing physical and mental health services to 1.6 million Puerto Ricans. Any person can voluntarily apply and be evaluated at the Local Offices of the Medicaid Program of the Department of Health for the Vital Health Plan of the Government of Puerto Rico. Benefits & Services
The Vital Plan offers you more benefits and services within a preferred network of providers from the medical group of your choice. Services are offered by networks of providers (primary care physicians, specialists, laboratories, etc.) across the island.
Learn about your benefits:
Extended hours at your Primary Medical Group: You have access to emergency services at your Primary Medical Group, until 9:00 p.m. Coordinate with your Primary Care Physician so that you know the availability of service during extended hours.
Your Primary Medical Group’s Preferred Network: With Vital, your Primary Medical Group will have a Preferred Network that includes specialist doctors, laboratories, X-rays, and hospitals, among others. You will receive your ID and a copy of the Providers Directory. Upon receipt, you will be able to visit any doctor or facility within your Primary Medical Group’s Preferred Network without needing a referral. You also won’t pay copays when using specialists within the Preferred Network.
Countersignature: You will not need a Primary Care Physician’s countersignature on prescriptions from the Preferred Network specialist.
Mental Health Coverage: Access to mental health services through a mental health professional at your Primary Medical Group facility.
