The Privacy Rule of the federal Health Insurance Portability and Accountability Act entitles Vital enrollees to have access to their medical utilization of services. Enrollees need their medical information for their own records and to share with health providers. Up-to-date record of services can help avoid duplication of medical tests and procedures, and can also help you get a better understanding of your health care.
It is easy to request your utilization report. Be sure to complete all the spaces requested. You will receive the information electronically. You may also have access to a printable copy of the report. If you want to have a paper copy of your information you shall contact call First Medical at 1-844-347-7800 or visit any of our Service Offices. First Medical will provide your report free of charges.
The Customer Service Department offers language and sign language interpreter services free of charge. This includes, service alternate formats such as; Braille, enlarged print and translation into other languages, verbal or written, among others. If you need plan information in another format or language, please contact our Customer Service Department Monday through Friday from 7:00 a.m. to 7:00 p.m. at 1-844-347-7800.
